All students entering the 12th and 7th grade in the fall of 2018 are now required by Ohio law to provide proof of the following immunizations BEFORE starting school in the 2018/2019 year:
Any student who has not had the Meningicoccal (MCV4) vaccination prior to entering the 12th grade will not be permitted to attend school after September 4, 2018 until proof of these immunizations has been received by the school.
Any student who has not had the TDap and Meningicoccal (MCV4) vaccination prior to entering the 7th grade will not be permitted to attend school after September 4, 2018 until proof of these immunizations has been received by the school.
If you have already turned in an updated vaccination form there is no further action required on your end. If your child's immunization records are not up to date, we encourage you to contact your primary care physician to schedule an appointment for the vaccinations as soon as possible. You may also contact the Wolves Wellness Center at West Clermont Middle School at 513-732-5082 to schedule an appointment. You may use your current health insurance at the Wolves Wellness Center. However, if you do not have health insurance the vaccinations will be provided by the Wolves Wellness Center at no cost to you. Vaccinations are also available at many local pharmacies.
During the remainder of July, please drop off the completed vaccination information to the West Clermont Local School District Office (4350 Aicholtz Road Suite 220).
