 Immunizations

   Pupils who have not received the minimum number of immunizations and are not otherwise exempt may remain in school only if they have received measles, mumps, and rubella vaccines and at least one dose of DtaP/DTP/DT/Td, Hepatitis B and at least one dose of polio vaccine.  They must finish the series as soon as the scheduled intervals between doses permits.  If you have any questions please call toll-free 1-800-282-0546.

Vaccine

Fall 2002 Immunization Requirements for Day Care, Head Start, & Pre-school Fall 2002 Immunization Requirements for School

DTaP/DTP/DT

4 doses of DTaP, DTP, or DT or any combination. Kindergarteners- 5 doses of DTaP, DTP, or DT, or any combination if the fourth dose was administered prior to their 4th birthday.

Grade 1-12- 4 doses of DTaP, DTP, or DT or any combination.

Polio

3 doses of OPV or IPV or any combination of OPV or IPV. Kindegarten- 4 doses if a combination or OPV or IPV was administered.  4 doses of all OPV or all IPV is required if the third dose of wither vaccine was administered prior to the 4th birthday.

Grades 1-12- 3 doses of OPV or IPV or any combination of OPV or IPV.

MMR

1 dose of MMR administered on or after the first birthday. K-3 and 7-12- 2 doses of MMR.  Dose 1 must be administered on or after the first birthday.  The second dose must be administered at least 28 days after dose 1.

Grades 4-6- 1 dose of MMR administered on or after the first birthday.

Hib

3 or 4 doses depending on the vaccine type and the age when the child began the first dose of Hib.

1 dose if given on or after 15 months of age.

None

HEP B

3 doses of Hepatitis B Kindergarteners-3- 3 doses of Hepatitis B.  The second dose must be administered at least 28 days after the first. The third dose must be two months from the second and at least four months from the first and administered on or after six months of age. 

Grades 4-12- NONE

Healthcare 

  Because your health is important to the overall well being of our community, a health benefits program has been developed to assist you in staying healthy or getting care when needed.  The program is called Clermont Family Health Connections.  This program is geared toward those individuals who are losing Medicaid benefits but others in different situations will be considered.
To be eligible for this program you (or the primary wage earner in your home) must:
Ø      Be working 30 hours/week or actively seeking employment (if attending school or participating in a jobs program, this will be counted toward worked hours).

Ø      Not have access to other health plan (insurance) through an employer or are on Medicaid.

Ø      Be at or below 200% poverty level

Ø      Be younger than 65 years old.

  The benefits of the program include your choice of a doctor form the panel, who will assist you in getting appropriate medical care when needed.  You will also be assigned a Health Partner who will review your overall health status and work with you to prevent illness and increase your overall wellness. The only cost to you will be a small co-payment for primary care visits and medication.  We urge you to take advantage of the plan BEFORE you really need it.

  Healthy Start and Healthy Families offer free and low-cost health care coverage to families, children (up to age 19) and pregnant women.  Coverage includes: doctor visits, hospital care, pregnancy related services, prescriptions, vision, dental, substance abuse, mental health services and much more.  Most families will receive coverage at no cost to them.  Some families, depending on family income, will be required to pay a small annual premium.  Healthy Start and Healthy Families are Medicaid programs administered by the Ohio Department of Job and Family Services.  For more information, please call
