Health records requirements for new enrollees

Kindergarten & 1st grade

     An immunization/physical form will be provided by the school when the child registers. The form must be signed by a physician MD.  The completed form must be then returned to school before the child is enrolled.

     3  - doses of  Hepatitis B

      4 - doses of  Polio -OPV or IPV( 4 dose are required if the 3rd dose was administered prior to the fourth birthday

      5 - doses of DTaP, DPT,or DT or any combination ( 5 doses are required if the 4th dose was administered prior to fourth birthday)

      2 - doses of  MMR (Dose 1 must be administered on or after the first birthday. The second dose must be administered at least 28 days after the first dose.)

      1 or more HIB

      TB Test (recommended)

 

  Grades 2 thru 12
